Template Overview
This employee evaluation form template gives managers and HR teams a structured, nine-step review funnel that rates an employee across six performance criteria, calculates a total score out of thirty, and generates personalized feedback with AI. It replaces static Word or PDF appraisal forms with an interactive digital experience that is easier to complete on any device and easier to analyze once the reviews are in.
The form opens with a personal information step (employee ID, department, reviewer name and title), then walks the reviewer through six rating-scale questions covering Job Knowledge, Work Quality, Attendance and Punctuality, Productivity, Communication and Listening Skills, and Dependability. Each rating step includes an open comments field so the reviewer can add context. The final page sums the six ratings into a score out of thirty using a built-in formula and displays AI-generated feedback tailored to that employee's specific scores and comments.
Because it runs on involve.me's no-code builder, you can change the rating criteria, adjust the scoring weights, rename departments, add self-review questions, or translate the form for international teams in minutes. Teams use this template for quarterly reviews, annual performance cycles, probation reviews, and 1:1 manager feedback - any scenario where a consistent, scored, documented evaluation is needed across the company.
Key Features
Six Built-In Rating Criteria - The template ships with six performance criteria on a rating scale: Job Knowledge, Work Quality, Attendance and Punctuality, Productivity, Communication and Listening Skills, and Dependability. Add, remove, or rename criteria to match your competency model without touching code.
Automatic Score Calculation with Formula Builder - A built-in formula sums the six rating answers into a total score out of thirty, displayed on the final page. Adjust weights for different criteria, add VLOOKUP rules to map score ranges to performance bands (Exceeds / Meets / Needs Development), or layer in custom logic.
AI-Generated Feedback per Employee - Personalized AI Text generates a unique narrative summary for every employee based on their specific ratings and open-comment responses. Each review ends with written feedback the manager can copy into a follow-up email or HRIS record instead of writing it from scratch.
Comments Field on Every Criterion - Each rating step has a paired open text field ("Comments for Job Knowledge", "Comments for Work Quality", and so on) so reviewers capture context, examples, and development goals alongside the numeric score. No more rating-only reviews that lack qualitative detail.
Multi-Step Layout with Conditional Logic - The review is broken into nine focused pages instead of one overwhelming single-page form, and Conditional Logic can show or hide specific criteria based on role (for example, "Leadership" criteria only for managers). Logic Jumps route reviewers to the right path based on answers.
Benefits
Consistent Reviews Across Every Manager: Every reviewer rates the same six criteria on the same scale with the same comment prompts, so scores become comparable across teams and departments. This is the main problem with free-text Word templates - one manager's "meets expectations" is another manager's "exceeds", and there is no way to normalize. A scored digital form fixes that.
Time Saved on Feedback Writing: The AI-generated summary on the final page means managers do not have to write a long narrative from scratch. They review the AI output, edit as needed, and share it with the employee. Reviews that used to take an hour each are completed in fifteen minutes, which is the difference between cycles that happen on schedule and cycles that slip.
Trackable Performance Data Over Time: Because scores and comments are stored as structured data in the Responses tab (not buried inside PDFs), you can export a spreadsheet of every review, compare an employee's score trajectory across quarters, and spot teams or criteria where scores are trending down.
A Professional Digital Experience for Managers and Employees: Reviewers complete the form on any device, progress is shown clearly, and the final screen presents the score and AI feedback in a polished, branded layout. Compared to a DOCX file passed around on Slack, the digital form feels like a real HR tool and signals that performance reviews are taken seriously.
Connects to Your HRIS, CRM, or Spreadsheet: Push every completed review into HubSpot, Salesforce, Google Sheets, Airtable, Notion, Slack, or Microsoft Teams through the Connect tab. Use Workflow Automation to email the employee their feedback the moment the reviewer submits, and notify HR when a review falls below a threshold.
How It Works
Customize
Open the template in the drag-and-drop editor and rename the six default criteria to match your competency framework (for example, swap "Job Knowledge" for "Technical Expertise" or add "Leadership" as a seventh criterion). Adjust the rating scale from 1-5 to 1-10 if you prefer, update the department list, and restyle fonts, colors, and the progress bar to match your brand.
Configure
In the Settings tab, turn on password protection so only managers with a link can access the review form, set a custom data retention period for HR compliance, and configure the AI feedback prompt with your preferred tone and length. Use Conditional Logic to show or hide criteria based on the employee's department or role.
Connect
In the Connect tab, map every field - reviewer, employee ID, department, the six scores, the six comments, the total score, and the AI feedback - to your HRIS, spreadsheet, or CRM. Native integrations include HubSpot, Salesforce, Pipedrive, Google Sheets, Airtable, Notion, Slack, and Microsoft Teams, with Zapier and Webhooks for anything else.
Share
Publish the review form as a standalone landing page with a custom domain, embed it inside your HR intranet or Notion workspace, or send the link by email to managers at the start of each review cycle. One funnel URL handles every review regardless of which channel the link came from.
Analyze
View every submission in the Responses tab with filtering, sorting, and XLS or CSV export. AI-powered analytics surface average scores by department, by criterion, and by reviewer - so you can see which managers are stricter graders, which criteria are consistently lowest, and which teams need development investment.
